Qualifications (but not limited to):

  • Required: Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice w/no limitations and restrictions.
  • Experience as a Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care or primary care.
  • Advanced knowledge and decision-making skills in assessment, diagnosis and health care management.
  • Knowledgeable about primary health care issues in Long Term Care.
  • Leadership knowledge/experience is considered an asset. Certifications obtained or in progress that include gerontology, mental health, behavioral support and palliative care are an asset.
  • Working Knowledge of the; Fixing Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, considered an asset

POSITION SUMMARY: not limited to

  • Providing primary care in accordance with established policy, the resident plan of care and within the scope of practice.
  • Collaborating with the physician to provide follow-up on diagnosed conditions and provides on-call support.
  • Formulates and communicates diagnoses, determines the need for medical orders and any necessary diagnostic tests, as per scope of practice, including labs, X-rays, ECG’s and ultrasounds, and interprets and acts on the results.
  • Maintains ongoing Resident documentation, including assessment findings, treatment plans and progress. Consistently records and documents in the patient’s electronic health record
  • Providing nursing consultation to residents and their families, health professionals, administration, groups or organizations.
  • Participating in the identification, analyzing and interpreting trends in resident care outcomes and professional nursing practice issues to determine priorities for educational programming.
  • Responding promptly to concerns of residents and family members, mediating a satisfactory response to concerns and implements action plans to avoid the recurrence of similar issues.
  • Performing other duties as assigned.
  • Arrange follow-up appointments as necessary. Referral/Consultation/Collaboration

REPORTING RELATIONSHIP: Reports to the Medical Director (MD) and Executive Director and collaborates with the Director of Clinical Services (DCS) at Wellesley or Director of Resident Care (DRC) at Sherbourne to help support and align with our resident care needs.

REQUIREMENTS FOR THIS POSITION INCLUDE:

  • Excellent written/ oral communication skills, with respect to staff, Residents, and Family Members
  • Positive working attitude with a strong work ethic
  • Experience with working with geriatric and Alzheimer’s/ Dementia population in a long-term care setting or the community
  • Physical fitness: Repetitive bending movement at/above/below waist/shoulders/ knees for extended periods of time. Pushing/ pulling movement, periodically lifting may be required, swift walking etc
  • Ability to maintain a calm and composed demeanour in a high attention demanding environment
    Job Type: Full-time
    Pay: From $130,000.00 per year
