About the Role We are seeking a curious and collaborative Nurse Practitioner to join our PACE IDT as part of a robust interprofessional care model. You’ll work hand-in-hand with PCP partners, nurses, social workers, rehab therapists, behavioral health specialists, dietitians, recreation therapists, chaplains, and others to deliver wraparound care that engages older adults as they age in place, on their own terms. This is a deeply relational, fulfilling role—you’ll build meaningful ties with your participants and with colleagues who are equally committed to high-integrity, person-centered care. What You’ll Do • Provide whole-person primary and supportive care in the clinic, home, and community-based settings. • Partner with participants and families in managing chronic and serious illness, goals of care, and care planning. • Co-lead IDT discussions, contributing your voice to shared decision-making. • Participate in quality improvement and professional development initiatives guided by physician mentors and coaches. • Foster trust, presence, and accountability across all disciplines. What You Bring • Current NP licensure (AGNP, FNP, GNP) and prescriptive authority in Colorado. • Experience in geriatrics, family/internal medicine - interest in palliative care a plus. • Strong clinical skills with a deep appreciation for interdisciplinary collaboration. • Excellent communicator, relationship-builder, and advocate for vulnerable populations. • Committed to shared learning, self-care, and care equity.
