Description Status: PRN / As Needed Location: Milwaukee County & Surrounding Areas Allegiant Hospice is a new, mission-driven hospice agency serving Milwaukee County. We are seeking a compassionate PRN Nurse Practitioner to join our growing interdisciplinary team. This role provides as-needed clinical support, patient visits, symptom management, and collaboration with our Medical Director and Director of Clinical Services. Requirements Key Responsibilities Provide PRN visits for hospice patients in homes, facilities, or community settings Support symptom management, medication adjustments, and patient/family education Complete face-to-face (F2F) encounters as assigned Document timely and accurately in the EMR Collaborate with the hospice team (RN case managers, social workers, chaplains, aides) Communicate changes in condition to the Medical Director and DCS Participate in IDG meetings as needed Qualifications Current WI Nurse Practitioner license (APRN) Active DEA or willingness to obtain Hospice or palliative care experience preferred (not required) Strong communication, critical thinking, and compassionate bedside presence Ability to work independently and provide timely visit availability Why Join Allegiant Hospice?
