We are seeking a highly skilled After-Hours Telehealth Nurse Practitioner to join our growing team. This flexible locum role allows you to work remotely while making a meaningful impact in aged care.

As an After-Hours Nurse Practitioner, you will:

• Provide advanced nursing assessment, diagnosis, intervention, and management to aged care residents via telehealth.
• Address acute health issues, update care plans, and coordinate care with facility staff, GPs, and specialists.
• Issue e-scripts, order/review investigations, and make referrals to telehealth specialists when required.
• Conduct after-hours telehealth rounds and ensure timely documentation to support continuity of care.
• Contribute to the ongoing improvement of our innovative after-hours model of care.
