Position Classification: Nurse Practitioner / Transitional Nurse Practitioner (TNP will be under a maximum of 3-year contract paid at Clinical Nurse Consultant Grade 2 whilst undertaking requirements to become an Endorsed Nurse Practitioner. On endorsement, the position holder will transition to NP Year 1).
Remuneration: $131,870.90 - $149,538.56 pa (+ superannuation + salary packaging options)

ABOUT THE ROLE

The Nurse Practitioner / Transitional Nurse Practitioner - Community Aged Care, provides outreach services into Aged Facilities in Bathurst, offering specialised advanced clinical care with a focus on hospital avoidance, reducing the number of emergency presentations, inpatient admissions & length of hospital stays. This role in an integral part of a multidisciplinary health care team liaising with ED, Palliative Care, Health in the Home & the inpatient aged care team.
