Description Nurse - RN- Full Time - Day Shift - Every other weekend The Clinical Support Team Nurses are responsible for monitoring the clinical status of the elders and delivering care. The Clinical Support Team is interdisciplinary and includes nurses, occupational/physical/speech therapists, social workers, dieticians, and activity directors. At most nurses are responsible for overseeing the care of elders living in 2 cottages (a maximum of 20 people) Functions of the job: Obtain an update on each elder from the Care Partner during each shift Visit each elder during each shift to monitor clinical status, and to determine if the clinical care needs of the elder are being met. Clinical assessments should be elder-specific, consider the primary diagnosis and risks specific to the individual elder. Assess each elder for unresolved/untreated pain. Plan and implement pain management interventions when appropriate, especially related to: skin integrity, bowel and bladder status, including continence and bowel movement monitoring, mentation, ambulation status, nutrition (weights) and hydration, and the effects of medication Identify any clinical teaching needs of the Care Partner, elder or family. Provide the teaching and complete the documentation Monitor standard precautions (i.e. presence of gloves, soap, towels, etc.) Administer treatments and medications Monitor overall adequacy of the care plan Review compliance with the care plan Maintain emergency supplies and equipment Conduct physical assessment of each elder In addition to their roles in providing clinical assessment, treatment, and evaluation for the elders, the CST members are also partners with the Care Partners and bring the following four professional roles in relationship to the Care Partners: Care role model – recognizing the strengths and abilities of each elder, supporting these strengths to minimize excess disability, and ensuring that care approaches are person-directed and allow the elder maximum control over his/her care and life. Erotological expert – bringing one’s skills, expertise and specialized training in assessment, care and treatment of aging people to bear on care and services provided in the cottage. Care team partner – As care team partners, all individuals involved in care must share information, listen to one another, share power and foster creativity. Mentor and teacher – acting as trusted guides and advisors to the Care Partners, as well as imparting knowledge and teaching skills. Job Type: Part-time Pay: $35 - $39.00 per hour Requirements Minimum of 1yr of Nursing experience Long Term Care experience Nursing licenses in good standing
