Description Nashua, NH Greater Nashua Mental Health is seeking a full-time Nurse - RN/LPN to join our Assertive Community Treatment (ACT) Team. Working under the Nursing Manager and ACT leadership, this role provides comprehensive nursing support, medication management, and client education in both the agency and community settings. Key Responsibilities • Provide daily medication management and education for ACT clients; maintain accurate and timely documentation. • Package and verify medications for delivery; coordinate daily delivery schedules. • Conduct nursing assessments, deliver long-acting injectable medications, collect lab specimens, and enter orders. • Complete documentation within 3 business days. • Reconcile 95% of prescribed medications after client hospital discharge and process refill requests within 2 business days. • Communicate with pharmacies, medical providers, and insurance companies regarding client needs. • Attend team meetings and support integrated care planning. • Provide nursing coverage across ACT teams and occasional coverage for CSS clients. • Perform other duties or special projects as needed. Qualifications • Bachelor’s degree in nursing. • NH Registered Nurse license (required). • 1–2 years of related nursing experience. • Knowledge of psychotropic medications; ability to safely administer IM injections. • Experience with EMRs; proficiency in Microsoft Office. • Strong communication, crisis intervention, and collaboration skills. • Ability to multitask, prioritize, and work effectively in a fast-paced environment. • Valid driver's license and personal vehicle with required liability insurance.
