Job Purpose: To provide advanced nursing care and support to patients in Primary Healthcare and Home Care Services. The Nurse Specialist will be responsible for delivering high-quality clinical care, promoting health education, and supporting the overall patient care plan in both settings. Key Responsibilities: Clinical Care: Provide advanced nursing care to patients, including assessments, and follow-up. Perform phlebotomy procedures and ensure proper handling of specimens. Perform specialized procedures as required, ensuring patient safety and comfort. Monitor and manage patients with chronic conditions, coordinating care with other healthcare professionals. Conduct home visits to provide personalized care to homebound patients. Patient Education and Support: Provide education to patients and their families on disease management, medication adherence, and lifestyle modifications. Provide support and counselling to patients and their families. Develop and distribute educational materials tailored to patient needs. Offer guidance on lifestyle changes and self-care strategies to manage chronic conditions. Coordination and Collaboration: Collaborate with physicians, specialists, and other healthcare professionals to ensure integrated patient care. Coordinate patient care activities, ensuring seamless transitions between primary care and home care services. Quality and Compliance: Adhere to all healthcare regulations, standards, and protocols. Participate in quality improvement initiatives and ensure compliance with safety and infection control standards. Maintain accurate and up-to-date patient records and documentation. Ensure compliance with home care regulations and standards. Professional Development: Stay current with advances in nursing practice and healthcare trends. Participate in continuing education and professional development activities. Qualifications: Education: Bachelor’s degree in Nursing from an accredited institution. Licensure and Certification: Valid nursing license in Saudi Arabia from the Saudi Commission for Health Specialities (SCFHS). Certification in a specialty area (e.g., Primary Care, Home Health) is preferred. Experience: Minimum of 3 years of clinical nursing experience. Experience in patient education and health promotion. Proficient in phlebotomy techniques and patient care. Skills: Excellent clinical and critical thinking skills. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Proficiency in electronic health records (EHR) and other healthcare technologies.
