We wish to recruit a B4 nurse associate to join the Older Adults CMHT in Lincoln & Gainsborough. The team provide support for older adults experiencing mental health difficulties and the assessment and treatment for patients of all ages presenting with memory difficulties and mental health needs. The team supports flexible working, a combination of office and home working.
To provide a high standard of clinical care ensuring safe and effective assessment, treatment (including depot injections) and comprehensive discharge planning using clinical reasoning skills and appropriate assessment tools. To plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk. Offering nursing interventions in a wide variety of clinical, therapeutic and social forms with the aim of promoting health, wellbeing, self autonomy and independence. To manage caseload and service delivery to contribute towards the attainment of Service/Trust productivity requirements. The post holder will have a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person cantered care and support for people of any age and in a variety of settings. To assess carers’ and families needs and develop, implement and review programmes of support for cares and families. Enable individuals to develop independent living skills and live in the community. The nursing associate will work independently under the leadership of the registered nurse, working within the sphere of nursing and care and within all aspects of the nursing process. To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Community Mental Health Team and with external agencies.
