POSITION SUMMARY:

Lead the front-line care team and promote and manage the nursing rehabilitation program in the Home in combination with the MOHLTC standards and regulations and preferences of the resident.

QUALIFICATIONS:

  • Registered Practical Nurse with a current registration and in good standing with the College of Nurses of Ontario
  • 3 years of experience in geriatric nursing with a genuine interest in all aspects of Long Term Care
  • Diploma/Degree in Rehabilitation, PTA and/or OTA or Kinesiology or related education is preferred
  • RAI/MDS proficiency is an asset
  • Clear understanding of Resident-Directed Care
  • Knowledge of LTC legislation, MOH compliance and Health and Safety
  • Excellent interpersonal, communication, organizational and team skills
  • Demonstrated experience and passion for positive leadership Computer skills (knowledge of Point Click Care, Microsoft Office)
  • Proficient in HQO principles and activities related to quality improvement activities including data collection, analysis, reporting, building QIPS.

Responsibilities
  • Responsible to lead the front line team in a designated resident home area communicating the goals and priorities of the organization, the vision and policies, empowering the team to deliver care and services in an inclusive work environment to achieve the best resident outcomes
  • Responsible to participate in all quality activities in the home in a collaborative and multidisciplinary environment
  • Coordinates Nursing Rehabilitation care programs with therapy service providers to provide transitional and complimentary services to residents
  • Coordinates and manages the required education to implement, sustain and improve the Home’s rehabilitation care programs
  • Manage resident and family concerns as they arise to avoid further escalation of issues
  • Manages and coordinates the planning, implementation, and evaluation of a wide range of rehabilitation care programs for eligible residents, including but not limited to Eating or Swallowing training, Walking Program, Range of Motion (active and passive), Splint or Brace Assistance, Bed Mobility, Transfer Training, Dressing or Grooming Training, Amputation or Prosthesis Care, Communication Training, Scheduled Toileting and Bladder Continence based on resident needs
  • Educates and supervises Team Members with implementing rehabilitation care programs and services
  • Maintains complete and accurate records of rehabilitation care program assessments and services
  • Will provide up-to-date rehabilitation assessment prior to scheduled care conference
  • Will develop personalized resident centered Rehab care plans, assessment, goal setting, interventions and evaluation
  • Assess residents related rehabilitation needs and strengths (new admissions and referrals)
  • Develop and coordinate nursing rehabilitation work routines for PCW’s for each home area and Rehab Assistants
  • Responsible to assist with hiring, performance management, disciplines and terminations of applicable team members within the department when required
  • Responsible to participate in, and onboard, newly hired PSW team members
  • Support and orientate the use of Point Click Care and Point of Care method of documentation
  • Provides leadership to the PSW group and is responsible for all HR related issues for the PSW group
  • Coordinate and lead PSW team meetings and daily home area huddles
  • Ensures completion of performance appraisals of direct reports (PSW’s)
  • Advocates for Resident Rights and helps to develop individualized Resident Care Plans
  • Assist in maintenance of records/documentation of resident’s condition, progress, care, specific instructions and evaluations
  • Leads and collaborates with Leadership Team concerning educational requirements within the Home and specific to the home area assigned
  • Keep up to date with new and current trends in nursing and Gerontology
  • Effectively leads the team by example and works collaboratively with the leadership team in achieving the goals and priorities established
  • Agile work hours to support the needs of the home as required, example evening hours and weekends
  • Support the MRC and the nursing department as required
  • Participates in Care Conferences as required
  • Other tasks as directed by supervisor
