The Nursing Restorative Therapist reports to the Director of Care and, under the direction of the Unit Supervisor, coordinates and supports the Home’s restorative care program to help residents achieve and maintain their highest level of function, mobility, and independence while preserving dignity and choice. As the Home’s designated lead for the restorative care program, the Nursing Restorative Lead ensures suitable residents are identified through assessment, restorative goals are reflected in the plan of care, and routine restorative nursing interventions are implemented consistently by the care team. Working closely with the Physiotherapist and interdisciplinary team, the Lead supports education, documentation standards, and ongoing monitoring of restorative outcomes to promote resident quality of life. Wage Rate: The starting wage rate is $26.75/hour. Vacancy: This is not an existing vacancy and is a new position. Responsibilities: * Coordinate the Home’s Restorative Care Program and ensure it is implemented consistently across units. * Establish clear restorative program workflows (referrals, eligibility, interventions, documentation, and follow-up) and ensure staff understand roles, responsibilities, and escalation pathways. * In partnership with the Unit Supervisor and interdisciplinary team, identify suitable residents for restorative programming using resident assessment information (admission, quarterly, and significant change assessments) and translate findings into restorative goals and care plan interventions. * Lead/participate in interdisciplinary care plan rounds as required, ensuring restorative goals are measurable, resident-directed, and reflected in the plan of care and daily routines. * Support accurate use of interRAI outputs (triggers, care planning priorities, outcomes tracking). * Oversee and support the delivery of routine restorative nursing interventions that promote function, mobility, and independence (e.g., ADLs, bed mobility, transfers, ambulation/walking, toileting/continence routines, strengthening/ROM as appropriate). * Ensure restorative approaches preserve the highest level of dignity, choice, and function, adapting interventions to resident goals, preferences, and abilities (person-centered practice). * Collaborate closely with Physiotherapy/OT (and other therapy partners) to identify residents who may benefit from rehabilitation services and to transition residents appropriately between rehab and restorative maintenance programs. * Work with the interdisciplinary team to modify restorative plans as needs change (posthospital return, significant change, new equipment, new risks). * Coach and mentor PSWs and care team members on restorative philosophy, techniques, and safe resident transfer/handling practices; observe practice and provide real-time feedback to support resident and staff safety. * Apply adult-learning principles to education (orientation, refreshers, skill reinforcement) and support competency tracking related to restorative practices and safe transfers. * Maintain high-quality documentation in the electronic health record (e.g., PointClickCare) including: care plan round notes, program eligibility/intake, quarterly reviews, multidisciplinary documentation, and participation/outcome tracking (monthly statistics and required reports). * Monitor documentation quality for accuracy, completeness, timeliness, and consistency with care plans; address gaps through coaching and follow-up. * Participate in the annual evaluation of the Restorative/Rehab Program, including reviewing outcomes, audit findings, resident feedback, and identifying improvement opportunities and action plans. * Participate in audits and quality monitoring related to restorative care (documentation audits, restorative participation, outcomes, safe transfer practices) and support improvement actions. * Understand and apply the Home’s quality indicators relevant to function and restorative programming; trend participation/outcomes and report themes to the Unit Supervisor/Director of Care to support CQI. * Protect resident personal health information and maintain confidentiality in accordance with PHIPA and applicable professional standards; model privacy-aware documentation and communication at all times. * Work in alignment with Occupational Health & Safety expectations, reporting hazards/near misses and supporting safe practice improvements, particularly related to resident mobility, transfers, and equipment use. * Support the accurate labelling of residents’ mobility aids and personal devices (e.g., walkers, wheelchairs, transfer belts, splints/orthotics, hearing/visual supports as applicable) to reduce loss, improve safety, and support consistent use. * Educates PSWs on Nursing Restorative Philosophy and techniques, and acts as a resource for PSWs * Apply and reinforce routine IPAC practices during all restorative activities, including hand hygiene, respiratory etiquette, appropriate PPE use, and safe cleaning/disinfection of shared equipment (e.g., walkers, wheelchairs, transfer devices) in accordance with Home policies. * Demonstrated knowledge of Point-of-Care Risk Assessment (PCRA) and ability to coach staff to apply PCRA to restorative activities (mobility, transfers, toileting, dining support) to determine appropriate precautions and PPE at the point of care. * Escalate IPAC concerns (symptoms observed during restorative activities, practice gaps, supply issues) promptly to the Unit Supervisor and/or IPAC Lead for timely follow-up. * Participate in the Home’s fire safety and emergency preparedness program, including attending required training, supporting drills, reinforcing safe movement/transfer practices during emergencies, and following established emergency codes and procedures. * Perform other duties as assigned and or indicated in Job Task Inventory Qualifications: The qualifications needed to join our family are as follows: * Post-secondary diploma, degree, or certificate in recreation and leisure studies, therapeutic recreation, kinesiology, gerontology, or a related field from an Ontario postsecondary institution. * Demonstrated experience in a long-term care, health-care, or other relevant setting, with the ability to coordinate resident programs and daily restorative interventions. * Strong commitment to person-centred care, supporting resident choice, dignity, independence, and quality of life. * Ability to develop, implement, monitor, and evaluate restorative programs/interventions based on resident assessment and care plan goals. * Demonstrated ability to coach/educate staff using adult learning principles, and to support consistent practice across shifts. * Strong documentation skills and comfort with electronic systems; ability to track participation/outcomes and maintain program records. * CPR/First Aid: Current CPR and First Aid certification. * Knowledge of interRAI, CAPs and care planning. AIS certified, if not must be completed successfully within one (1) year of being hired * Experience leading restorative approaches (mobility, transfers, ROM, functional maintenance, falls prevention supports, continence promotion, adaptive/assistive strategies) in collaboration with nursing and therapy partners. * Strong communication and interpersonal skills; able to build positive relationships with residents, families, and interdisciplinary team members. * Demonstrate strong computer skills and confidence using an Electronic Health Record * (EHR) (e.g., PointClickCare) to document restorative interventions, maintain accurate records, and support care planning. * Produce routine reports and statistics using EHR reporting tools and standard office software (spreadsheets, Word, email) and share insights with leadership to support quality improvement. * To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction * To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk * Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one * As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements * Two supervisory references required * Provide proof of all required vaccines.
