JOB DESCRIPTION SUMMARY

Provides leadership for a team of both professional staff and nonprofessional staff. Oversees the delivery of quality care to patients and families on respective teams. Supervises, guides and directs staff in carrying out their roles within the agency. Works collaboratively with the Clinical Director, HR Director, Executive Director, and other management personnel to facilitate the development and implementation of quality agency services. Works collaboratively with other community agencies. Also, provides nursing care according to the Community Health Nurse job description.

Responsibilities
  1. Meets with team members to review their caseloads and to assess the staff member’s ability to effectively manage their caseload.
  2. Makes on-site supervisory visits with team members when appropriate or requested by staff.
  3. Works collaboratively with Clinical Director, Chief Executive Officer and other management personnel for agency planning and problem solving.
  4. Assists with recruiting, hiring of new staff, orientation of new staff, and assignment of new staff.
  5. Assign new referrals and unassigned re-visits as needed.
  6. Assists staff in planning caseload management and/or setting priorities for paperwork if appropriate.
  7. Possesses knowledge of Medicare and Medicaid regulations to ensure compliance by staff.
  8. Possesses a knowledge of third-party billing requirements.
  9. Plans and directs team meetings.
  10. Assists in planning for clinical staff education programs and encourages continuing education courses and seminars.
  11. Assists in the development and interpretation of the philosophy and objectives of the agency.
  12. Facilitates positive interpersonal relations among staff.
  13. Serves as backup in the absence of the clinical director.
  14. Assists staff in problem solving and in establishing plans for the continuity of care.
  15. Participates in the agency’s quality improvement program.
  16. Evaluates and works with the Clinical Director on performance review of each team member annually.
  17. Performs clinical skills competencies annually.
  18. Responsible for on-call back up and vacation coverage
  19. Assist with weekend schedule.
  20. Conducts and works with Clinical Director and HR Director in the termination process of low performing staff members.
  21. Keeps abreast of home health and management issues, as appropriate.
  22. Other duties as assigned
