Description SUMMARY: We are currently seeking a motivated individual to coordinate the Food and Nutrition services for adult patients. Working closely with providers, this position will promote nutritional programs that are tailored to a culturally diverse clientele. Responsibilities include providing individual and group counseling, assessing nutritional needs, suggesting diet modifications, conducting educational nutrition programs in the community and providing referrals to outside agencies and programs, as needed. Under the direction of the Chief Medical Officer, the Nutritionist plans, organizes and provides nutrition services GENERAL DUTIES & RESPONSIBILITIES: Clinical: · Works with administrative and health care providers in defining and establishing appropriate standards, policies and criteria for nutrition services · Provides individual and group counseling in therapeutic and normal nutrition practices · Conducts educational programs to foster proper nutrition and food education in the community · Obtains patient’s diet history and assesses nutritional status · Advises and helps motivate patients to make dietary changes to alleviate or control medical conditions, such as diabetes, hypertension, obesity, hypercholesterolemia and for normal nutrition practices such as prenatal diets, infant and child feeding, and diet for optimal health · Consults with health care providers regarding specific patient cases and nutrition care · Provides follow-up and referrals for other services when appropriate · Document service in medical record · Organizes plans and conducts group education classes as appropriate or requested by other departments on topics such as weight loss, hypertension, cholesterol and diabetes · Obtain or prepare audio-visual aids, posters, brochures, etc. to assist such classes General Office Responsibilities: · Coordinates with federal WIC program · Screens prenatal patients for WIC eligibility and complete WIC nutrition assessment form · Refers to food assistance programs and food pantries when appropriate. · Work with outside agencies such as elderly, schools and community groups to establish needs and coordinate programs · Ascertains and provides information to improve nutrition practices in the community. May assist with food budgets · Provides nutrition information for medical providers and staff · Obtains develops and updates nutrition information materials needed for patients · Adapts informational materials to cultural and ethnic preferences of community groups, ensuring translation of materials into appropriate languages. Provide educations for resident program, participate in agency staff meetings and patient conferences Requirements REQUIRED SKILLS & COMPETENCIES: Education: B.S. in Nutrition or equivalent Experience: · Registered Dietician. · Three years of community experience preferred · Proficiency in Vietnamese, Spanish, Cape Verdean or Haitian Creole, preferred. · Experience with Epic EMR preferred.
