Description ESSENTIAL JOB RESPONSIBILITIES: Completion of quarterly visits, observation and monitoring, and monthly contacts. Conduct iHC assessments according to OAAS’ timelines and enter them into the data system within 10 days. Conducts and completes annual CPOC according to OASS’ timelines. Submitting plans to the Support Coordinator Supervisor for approval according to OAAS and ESL timelines. Planning, scheduling, and coordinating quarterly meetings with participants and providers. Responding to participant changes in needs, completing/submitting appropriate paperwork when a revision is needed. Researching, accessing, and linking services. Communicating and working with providers to ensure appropriate service provision. Maintaining all responsibilities and requirements regarding critical incidents as required by OAAS Meeting with supervisor for weekly supervision, consultation, and participant needs. Attending and participating in weekly staff meetings. Attending and participating in required training. This position is a mandated reporter. Any mandated reporters shall make reports immediately upon learning of incidents of abuse or neglect to Adult Protective Services and Elderly Protective Services. Requirements QUALIFICATIONS: Must have a Bachelor’s Degree in Social Service or Human Services related field. Must have own reliable transportation. Must be willing to travel during the day to outlying parishes as required. Must be able to multi-task and meet deadlines. Must have excellent written and verbal communication skills. Must be proficient in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate. ? Must be a team-player. PHYSICAL REQUIREMENTS: The employee is regularly required to operate a computer, file and retrieve written documents, communicate with others on the phone and in-person. The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items. Ability to move independently or with reasonable accommodation within the facility and community. Must be able to travel and meet with participants. WORKING CONDITIONS: Work is performed in a normal working office setting that is environmentally controlled and out in the field.
