Franciscan Health Dyer Campus 24 Joliet St Dyer, Indiana 46311 The Occupational Therapist I provides evaluation and treatment of the patient within the parameters of legal and professional guidelines and hospital and departmental policies and procedures. The position instructs and directs Occupational Therapist Assistant and aides as assigned. The Occupational Therapist evaluates and implements the plan of care to improve, restore, and enhance performance in activities of daily living and functional mobility. This position performs documentation and quality improvement as outlined in departmental policies and procedures. WHAT YOU CAN EXPECT Patient Care: • Occupational Therapy Evaluations: Performs comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning. • Treatment Plan Development: Develops a treatment plan with measurable goals based on patient assessment. • Goal Re-Assessment and Discharge Planning: Completes appropriate goal re-assessment and discharge planning. • Provides occupational therapy, including selection and use of assistive equipment. • Trains and educates patients, family members and/or others to ensure understanding of treatment plan and goals. Patient Records: Documents and submits patient reports and charges within the established timeframe. Coordination of Care Collaboration: Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Supervises Occupational Therapy Assistants and Rehab Techs.
