As an Occupational Therapist (Status: Temporary, Full-Time; Start date: Immediately to December 18,2026 Location: Sudbury), you are responsible for the provision of occupational therapy services to Ontario Health atHome patients in order to maximize the patient’s function, independence, and safety in their respective environment.

• Assesses the factors influencing the patient’s ability to function in their environment, using a variety of standardized and non-standardized assessment tools.
• Determine patient centered therapy goals and provides treatment program to increase occupational performance, independence, and safety.
• Share knowledge and information with other health care professionals and agencies to work collaboratively to deliver effective and efficient patient care.
• Builds therapeutic relationships by ensuring patients’ values and preferences are respected and balancing patient needs and choices with available resources.
• Documents assessment findings, goals, and intervention plan in accordance with Ontario Health atHome policies and the College of Occupational Therapists of Ontario standards for record keeping.
