Under the direction of the Manager of Clinical Practice, you will provide client-centered care in the home and community setting, helping individuals achieve their highest level of independence and quality of life. Key Responsibilities (Depending on your assignment) Clinical Responsibilities: Conduct home safety and environmental assessments and develop individualized treatment plans, goals, and discharge strategies that aligns with best practice Maintain accurate clinical documentation in accordance with the College of Occupational Therapists of Ontario Collaborate with interdisciplinary health teams to ensure comprehensive care Communicate treatment recommendations and provide education to clients and caregivers Supervise and guide support personnel involved in therapy programs as appropriate Complete documentation and submit written reports to funder in accordance to contractual timelines Liaise with community stakeholders to assist client in renting or obtaining necessary medical equipment, and complete funding application as necessary Other Responsibilities: Maintain active registration with the College of Occupational Therapists of Ontario along with valid professional liability/malpractice insurance Mentoring or precepting OT students Participate in ongoing professional development Adhere to Bayshore policies and procedures Contribute to quality improvement initiatives Promote health and safety practices and report any concerns Maintain confidentiality of client and corporate information Perform additional tasks as assigned Qualifications Master’s Degree (or equivalent) in Occupational Therapy Registered and in good standing with the College of Occupational Therapists of Ontario Current professional liability/malpractice insurance Experience: 1–2 years of clinical experience practicing Occupational Therapy (in-home care experience is an asset) Assistive Devices Program (ADP) Authorizer status is considered an asset; willingness to pursue with evidence of pursuit to obtain ADP authorizer status is also considered an asset Experience supervising and delegating treatment plan to physiotherapy/Occupational Therapy Assistants (PTA/OTA) is considered an asset Skills & Abilities: Strong interpersonal and communication skills (oral and written) Ability to work independently and collaboratively within a team Comfortable in a fast-paced, client-focused environment Ability to manage challenging situations with professionalism Proficient with personal computers and secure email systems Self-directed with a commitment to continuous learning Additional Requirements Valid driver’s license, auto insurance, and access to a reliable vehicle Clear criminal background check with vulnerable sector screening Proof of TB clearance Two supervisory references New Graduates Are Welcome to Apply!
