Description:

The Harrow Community Therapy Service is looking for a suitable newly qualified or returning to practice candidate who is looking to develop their knowledge and skills within a friendly team. Support is readily available and a supervision and CPD structure is in place and encouraged by the service.
You will receive professional guidance and direction from senior Occupational Therapists and Physiotherapists. As part of your development, there are opportunities to experience colleagues working in a range of partner settings, and to develop your supervisory skills with Therapy Support Workers and students.
You will be working closely with the other internal Integrated and Specialist Community Services, including, Community Nursing, Speech and Language, Tissue Viability, Falls and Rapid Response services. You will also be required to work closely with Primary Care Network Colleagues, Frailty Service and the Hospital Discharge Team to support hospital discharge
You will be required to complete electronic patient records (training provided)
The position is full time (37.5 hours per week) but in line with our flexible working approach we welcome applications for part time contracts. A seven-day working pattern will be required to cover weekends and bank holidays on a rostered/rotational basis.

To provide therapy assessment and treatment for referred clients, as part of a multi-disciplinary team, planning, implementing, and evaluating interventions for groups or individuals in conjunction with senior therapists.
To undertake a variety of rotations (Unplanned Care Short Term Rehab/ discharge to assess/ Intermediate Care/ Triage/ Other pathways as required, to provide a broad base for both personal and professional development. This will involve working in several settings including care homes, residential units, patient homes.

Responsibilities:

To assist Specialist Physiotherapist in supervising, providing planned advice, teaching, support, and instruction to therapy assistants, key workers, relatives and carers, to promote understanding of the aims of therapy intervention and to ensure a co-ordinated approach to patient care.
To delegate to, teach and supervise the work of Therapy Assistants and Technical Instructors with guidance from senior staff.
To work with other health care professionals on joint assessments, patient goals and treatment programmes in a patient centred approach.
To undertake comprehensive assessment independently, plan and formulate an individual management and/or treatment programme, using clinical reasoning methods incorporating skills to implement therapeutic intervention
