Description

An Occupational Therapist uses their knowledge and skills of occupation to provide assessments and therapeutic activities for individual and/or groups of patients .

THE POST HOLDER WILL:

Provide a personalised intermediate care service to adults in their own home within Airedale, Wharfedale and Craven areas. This also includes providing therapy support to patients in Castleberg Hospital.
Perform occupational therapy assessment of patients with a diverse range of presentations and complicated physical and psychological conditions, to provide a diagnosis, develop and deliver an individualised treatment programme in conjunction with the rest of the team.
Provide a key role in working in partnership with medical colleagues, multidisciplinary teams (MDT) and carers to set realistic objectives and to help to facilitate goal and discharge planning to enable safe discharge from our service.
Work to support the team in its delivery of interventions, to skill share with other professionals to support the full breadth of the service (competency training is offered to support this skill sharing method of delivering intervention).
This is a community post therefore you will be required to have a driving licence and access to a car
