The Occupational Therapist is responsible for providing consistent and high quality therapy services, including evaluations and treatment for inpatients, outpatients, skilled nursing facility patients, home health patients, and other clients as necessary. Our core values are practiced and exhibited throughout the organization in our actions and in services provided.Joy : Unity : Integrity : Compassion : Excellence Essential Job Functions Works with patients and family members utilizing evaluations and treatment techniques specific diagnostic categories and/or functional impairment. Provide individualized intervention and education utilizing task analysis based on identified learning needs of the patient to ensure optimal patient care. Responsible for completing documentation within established department standards to enhance effective patient care and to proper reimbursement. Records billing information of services provided. Interacts and practices in collaboration with a variety of professionals and health care consumers. Participates in departmental and multidisciplinary review of process, treatment or system issues for Quality Improvement initiatives. Assists with cleaning and maintenance of treatment area and department. Assists with maintaining inventory of stock and placing orders for the department. Complies with department competency list. Pursues relevant education opportunities to maintain or enhance professional knowledge and to meet departmental needs. Provides services in multiple settings (including skilled nursing, assisted living, patient homes, schools and outpatient) as needed or directed. Performs individualized activity assessments and follow-up visits with each swing bed patient to determine their interests and abilities and to develop an activity care plan. Other job duties and responsibilities as assigned to effectively meet the needs of the patients, the department, and the organization as a whole. Requirements Education Master’s Degree in Occupational Therapy unless grandfathered in at the Bachelor’s Degree Level: Required Computer Skills Proficient in Microsoft Office Certifications & Licenses Current State of Wisconsin License as an Occupational Therapist: Required Current State of Iowa License as an Occupational Therapist : Required
