QVM is a 21 bedded community hospital providing Rehabilitation for a wide range of conditions (medical, orthopaedic, neurological, falls, frailty) in a warm and friendly environment.
You will carry a caseload of patients as part of the inpatient therapy team, guiding their assessment, treatment, and discharge planning on a 17-42 day admission pathway.
- To undertake a significant clinical caseload to a high professional standard, using specialist knowledge and skills from theoretical and practical experience.
- To be responsible for the supervision, education and assessment of junior staff, technical instructors and students.
- To communicate with relevant medical and nursing staff, social workers, other health care professionals, patients, relatives and carers.
- To actively participate in Continuous Professional Development and departmental educational and training activities.
- To carry out accompanied home visits and unaccompanied access visits to assess the patients’ functioning in their own environment and communicate with relatives and carers regarding further care and/or treatment at home.
- To participate in departmental audit and research as required.

- To be professionally and legally responsible and accountable for, all aspects of your own work. To ensure a high standard of clinical care for the patients under your management.
- To hold clinical responsibility for a designated caseload of patients, and to organise this efficiently and effectively regarding clinical priorities and use of time, working without direct supervision.
- To independently undertake comprehensive and where applicable, standardised assessments of patients looking at all Occupational Performance areas including self-maintenance, productivity and leisure.
- To formulate soundly reasoned prognoses and recommend best course of intervention, developing patient centred goals to address occupational performance and skill deficits, enabling patients in the areas of self-maintenance, productivity and leisure.
- To facilitate educational and rehabilitative group sessions for selected patients.
- To undertake the continuous assessment and evaluation of the treatment programmes and modify the programmes according to the individual patient needs.
- To be actively involved in patient discharge planning.
- At all times to be aware of the needs of the patients and their carers and to promote and encourage patient independence and where possible restore functional ability.
- To assess and manage clinical risk within own patient caseload.
- To maintain comprehensive and accurate assessment and treatment records in line with legal and departmental and Trust requirements.
