This is a unique opportunity for an Occupational Therapist looking for a varied and autonomous role in Community Aged Care on a Full-time or Part-time permanent basis.
Your role will include visiting clients in their homes, providing comprehensive Occupational Therapy (OT) assessments and interventions that promote health, wellness and independence whilst supporting them to continue living safely in the community.

Responsibilities

DAY TO DAY THIS ROLE LOOKS LIKE:

  • Conduct comprehensive OT assessments for clients, including those with multiple and complex needs.
  • Provide specific OT services and programs, including home assessments and advice on activities of daily living with recommendations involving home modifications, equipment prescription, care planning and education
  • Facilitate and assist in development of a range of community based Health and Wellness programs and groups
  • Involve clients and carers in developing care plans that are person centred and reablement focused.
  • Provide support, education and direction to community workers and staff to assist them to best support our clients
  • Submit individual applications for home modifications and equipment in accordance with program guidelines
  • Work collaboratively with the wider allied health team, including nurses, physiotherapists, fitness instructors and allied health assistants
  • Face to face clinical work and minimum travel time between clients with time allocated for administration and reporting
  • Ongoing liaison with external agencies and suppliers to arrange services and products required by the clients in accordance with policies and procedures

TO BE SUCCESSFUL IN THIS ROLE YOU’LL NEED:

  • Tertiary qualification in Occupational Therapy with post graduate experience
  • Current AHPRA registration
  • Commitment and enthusiastic approach towards working with the clients, families, carers and advocates.
  • Experience in equipment prescription, pressure care, home modifications and falls prevention for older adults
  • Industry knowledge of local and international wellness and reablement models of care
  • Experience working in a community setting and demonstrated knowledge of the aged and disability service system in the community health sector - desirable but not essential
  • Current Drivers Licence and registered and insured motor vehicle
  • Willingness to apply for a Police Check
