Description Reliable Home Care Providers, Inc. is seeking a licensed Occupational Therapist passionate about serving individuals in their homes. This part-time position is ideal for a clinician seeking to gradually build their caseload with the support of an experienced and responsive administrative team. The role offers scheduling flexibility and the opportunity to work with a diverse patient population. Primary Responsibilities: Provide direct, skilled occupational therapy services in patients' homes Conduct evaluations, develop individualized care plans, and establish patient-centered goals Deliver therapeutic interventions aimed at improving functional independence Collaborate with physicians, case managers, and family caregivers to ensure continuity of care Complete timely, compliant, and accurate clinical documentation in accordance with regulatory standards Requirements Preferred Candidate: Licensed Occupational Therapist in the State of Illinois (required) At least 1 year of home health or community-based therapy experience preferred but not required. Familiarity with electronic medical record (EMR) systems (WellSky Home Health) Strong clinical judgment, communication skills, and commitment to professional standards Compassionate, punctual, and able to work independently Must have a valid driver’s license, reliable transportation, and proof of auto insurance.
