OCCUPATIONAL THERAPIST JOB TITLE: Occupational Therapist DEPARTMENT: Home Health/Hospice FULL TIME/PRN: PRN JOB SUMMARY The Occupational Therapist will play a critical role in enhancing the quality of life for patients by developing and implementing personalized therapeutic interventions that promote independence and functional ability. This position focuses on assessing patients with a variety of physical, cognitive, and emotional challenges and collaborating with multidisciplinary teams to design and implement rehabilitation plans using evidence-based practices. MINIMUM QUALIFICATIONS * Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution * Valid and current state licensure * Current CPR certification PREFERRED QUALIFICATIONS * Experience in rehabilitation settings * Familiarity with assistive technology and adaptive equipment ESSENTIAL DUTIES & RESPONSIBILITIES * Conduct comprehensive patient assessments to evaluate physical, cognitive, and emotional abilities and limitations * Develop and implement individualized treatment plans targeting rehabilitation goals * Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes * Monitor patient progress and modify treatment plans as needed * Educate patients and families on therapeutic techniques and adaptive equipment * Ensure timely and accurate documentation and compliance with policies * Maintain positive working relationships with facility staff * Perform other duties as assigned PHYSICAL REQUIREMENTS * Ability to sit, stand, walk, and perform physical activities required for patient care * Ability to lift, move, and transport patients with or without reasonable accommodation * Visual, hearing, and communication abilities sufficient for patient care All candidates must be able to perform the essential functions of this position.
