Job Summary We are seeking skilled and compassionate Occupational Therapists (OTs) to join our Home Health team across multiple counties in Michigan. This role is ideal for clinicians who want flexible part time work, autonomy in the field, and the opportunity to help patients restore function, improve safety, and enhance their overall quality of life at home. As a Home Health OT, you will perform evaluations, develop individualized treatment plans, deliver therapeutic interventions, and guide patients toward achieving functional goals. Competitive per visit rates with room for negotiation based on experience and coverage Flexible scheduling and the ability to select preferred counties or regions Strong clinical and administrative support team Consistent patient volume provided across all five counties Opportunities for continued professional growth within home health Responsibilities Conduct comprehensive occupational therapy evaluations in the patient’s home Develop individualized treatment plans focused on improving functional independence Provide therapeutic interventions including ADL training, strengthening, and safety education Recommend adaptive equipment and provide training as needed Document all evaluations, treatments, and recommendations accurately in the EMR system Communicate progress updates and changes in condition to the RN Case Manager and other team members Collaborate with physical therapists, nurses, and other clinicians to ensure integrated care Maintain compliance with home health regulations and agency policies Qualifications Qualifications Active, unrestricted Occupational Therapist (OT) license in the state of Michigan Minimum 2 year of home health or outpatient rehabilitation experience preferred Strong clinical reasoning and assessment skills Familiarity with EMR documentation Reliable transportation for field visits Ability to work independently while maintaining high care standards Background Screening Safe Hands Home Health Care conducts background screenings through Michigan LARA upon acceptance of a contingent job offer.
