Occupational Therapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark Schedule: Flexible schedule Full-Time/ Part-Time/ Casual (5-40 hours/week) Location: Oshawa, ON Sign-on bonus up to $5000.00 available! Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Occupational Therapists to provide services to our Adult clients within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size. OT Responsibilities: * Provide ministry funded, community based Occupational Therapy services within the client’s place of residence and/or through virtual services * Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, stroke, etc. * Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers * Assign patient care plan, and provide supervision and monitoring of occupational therapy assistants when appropriate * Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate * Complete funding applications as applicable * Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards * Opportunity to service private clients Requirements: * Registered Occupational Therapist with the College of Occupational Therapists of Ontario (required) * Experience working within a community setting is an asset but new graduates are welcome * Ability to work independently and as part of an inter-professional team * Ability to prioritize, manage self-scheduling and meet reporting deadlines * Excellent oral, written and report writing skills * Proficiency with computers * Strong interpersonal and organizational skills * Reliable transportation is required for this position We also offer: * Flexible health benefits – medical, dental, and vision coverage for you and your family. * Paid wellness and vacation days. * Employee Share Ownership Program (Shoppers Drug Mart) – a 25% company match of up to 5% of your base salary. * 30% employee discount at Shoppers Drug Mart. * Corporate discount at GoodLife Fitness, Workperks, and employee assistance program. * Unlimited referral rewards – up to $5000 per successful hire.
