Description

Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in–home and school-based services, including in–home and school-based programs to children, youth, families within the community.
Lynwood Charlton Centre is the Lead Agency for the Hamilton service area for Moving on Mental Health. This is a Ministry initiated plan to transform the experiences of children, youth and families dealing with mental health issues, and strengthen the community-based system for delivering mental health services to them. Through our Lead Agency role, we are responsible for leading and facilitating system reform in Hamilton. To do this, we work collaboratively with other child and youth mental health service providers, other affiliated community partners including our child welfare, health, and school board partners, and parents and youth engaged in our services.
The Centre is funded and licensed by the Ministry of Health (MOH) and the Ministry of Children, Community and Social Services (MCCSS) and accredited by the Canadian Centre for Accreditation. We are currently looking for permanent full time Occupational Therapist to join our team.

EXPERIENCE /SKILLS AND OTHER REQUIREMENTS OF OCCUPATIONAL THERAPIST

· Degree in Occupational Therapy from a recognized university
· Registered and in good standing with the College of Occupational Therapists of Ontario (COTO)
· Knowledge of pediatric mental health, dual diagnosis, and/or complex needs
· Experience in the field of Occupational Therapy
· Excellent assessment, formulation and intervention skills and direct experience assessing children’s occupational needs and developing intervention plans.
· Experience using standardized assessment tools (i.e., Movement ABC-2, , Sensory Processing Measure (SPM), Sensory Profile (SP), Beery VMI).
· Experience working with and developing intervention plans aimed to enhance client functioning particularly as it relates to individuals with developmental disabilities and complex mental health issues.
· Experience providing both individual and family as well as group programming.
· Minimum G driver’s License

Responsibilities

ROLE OVERVIEW

Working under the direction of the Clinical Director/ Senior Manager of Complex Needs, the Occupational Therapist works as a team to effectively manage cases as assigned, including fulfilling case coordination, consultation and providing direct service to children/youth and families

MAIN DUTIES AND RESPONSIBILITIES

· Help team members to identify clients whose sensory processing, motor and/or executive functioning may be impacting their functioning.
· Aid in the development of differentiated instructional, environmental and behavioral strategies. This will involve the collaboration with children, youth, their families and team members.
· Make recommendations for materials, tools and/or accommodations to facilitate functioning. This could also include the modification of the home/residence or client’s classroom environment.
· Enhance the interdisciplinary team’s capacity through in-services and provision of resources. In-services will be designed to inform interdisciplinary team members of additional causes of functional and behavioral issues (ex. sensory processing disorder, executive functioning and motor coordination challenges) as well as to inform team members of plausible strategies that they could implement to overcome the challenges of the students.
· Collaborate with the interdisciplinary teams DSW/CYW prime workers to help children, youth and families to access appropriate community resources.
· Contribute to and monitor overall treatment/support planning and interfacing with the health, education, mental health sectors and program managers.
· Case Management responsibilities
· Providing Therapy and Direct Care
· Facilitating assessments for children/youth and families referred trough the consultation model.
· Responsibility for contributing to overall case planning in collaboration with the interdisciplinary team.
· Acting as a resource and support person to DSWs, CYWs and Resource Workers, as needed.
· Collaborating with the Clinical Director regarding assessment and consultation duties.
· Recommending and referring for any additional services and supplemental assessments deemed necessary.
