The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Responsibilities
  • Qualifications in Occupational Therapy and current registration with the Occupational Therapy Board of Australia and the Australian Health Practitioner Regulation Authority (AHPRA).
  • Demonstrated clinical experience in the delivery of Occupational Therapy services for clients with chronic, aged and complex needs, designing and implementing client centred plans, managing complex client care and discharge planning.
  • Demonstrated experience in assessment, planning and evaluating interventions, with the ability to exercise independent professional judgement on routine matters with minimal direct supervision.
  • Demonstrated ability to provide clinical supervision to students and support other staff.
  • Demonstrated, effective organisational and time management skills.
  • Demonstrated effective written and verbal communication skills with consumers, teams and agencies, with the ability to work in multidisciplinary teams.
  • Demonstrated commitment to professional development and demonstrated knowledge and experience with quality improvement and evidence based practice.
  • Current Driver’s Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
