The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

This temporary part-time opportunity (17 hours per week) offers the chance to make a lasting impact, working alongside a dedicated group of professionals to deliver personalised care to clients facing complex conditions.
You’ll take the lead in assessing needs, setting goals, and delivering tailored treatment plans within a collaborative multidisciplinary framework. From pressure injury prevention and equipment prescription to home modifications and palliative support, your expertise will help restore function and ensure safety for individuals across their homes and local health centres.
