Job Description: Occupational Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase. Evaluate patients and perform appropriate therapy interventions Develop a comprehensive plan of care and coordinate multi-disciplinary care Perform prompt clinical documentation and visit notes Continued communication with physician, care team members, patient and family Comfortable working in all aspects of the home health setting Manage patient treatment and outcomes directly, or through COTA supervision Requirements: Strong organization skills and time management Able to multitask and accomplish and meet timelines Professional appearance and effective communication skills Current Texas Occupational Therapist (OT) license One year of occupational therapy experience Current Texas driver's license CPR certification Experience with healthcare software (Homecare Homebase preferred) Previous Home Health experience is helpful, but not required Schedule: Monday to Friday License/Certification: BLS Certification (Required) Occupational Therapy License (Required) EMR Used: Homecare Homebase Benefits: 401K Dental insurance Flexible schedule Health insurance Paid time off Referral program Travel reimbursement Vision insurance Work Location: On the road
