Occupational Therapist (OT) – Long-Term Care LP Therapy LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning. Responsibilities: review medical records; evaluate patient abilities and needs; develop and implement individualized treatment plans; provide ADL training, adaptive equipment, and environmental modifications; document progress per Medicare guidelines; collaborate with interdisciplinary teams; supervise COTAs as assigned; support patient satisfaction and quality outcomes. Qualifications: Bachelor's degree in Occupational Therapy; current state OT license; LTC experience preferred; strong knowledge of Medicare documentation; excellent communication and organizational skills. Work Environment: LTC facility; physical demands consistent with therapy services.

Responsibilities

The Occupational Therapist will evaluate patient abilities and needs, develop and implement individualized treatment plans, and document progress according to Medicare guidelines. They will also collaborate with interdisciplinary teams and supervise Certified Occupational Therapy Assistants as assigned.
