Description Facility: Sylva Valley Health & Rehab (Skilled Nursing Facility) Job Type: Full-Time (Part-Time/PRN considered) About the role Sylva Valley Health & Rehab is hiring an Occupational Therapist (OT) and/or Occupational Therapy Assistant (OTA) to provide high-quality therapy services in a skilled nursing setting. This role is ideal for clinicians who are passionate about helping residents improve independence, safety, and quality of life through meaningful, functional goals. What we offer Competitive pay (based on experience) Flexible schedule options (discussed during interview) Collaborative rehab environment and supportive team culture Vacation time + Sick time (per eligibility) Key responsibilities (OT) Evaluate residents and develop individualized occupational therapy plan of care Provide skilled OT interventions focused on ADLs, transfers, safety, cognition, and adaptive equipment Document evaluations, progress notes, and outcomes to support skilled coverage and care needs Coordinate with PT/SLP, nursing, providers, and the interdisciplinary team Educate residents/families on safety strategies, home programs, and equipment Participate in care conferences/IDT and discharge planning as needed Key responsibilities (OTA) Provide OT treatment interventions under the supervision of an OT Document treatments and resident response/progress Communicate changes in condition to the supervising OT and care team Assist with resident/family education per plan of care Support discharge planning and continuity of care Requirements OT: Current Occupational Therapist license + graduate of an accredited OT program OTA: Current Occupational Therapy Assistant license + graduate of an accredited OTA program Strong documentation and communication skills SNF/rehab experience preferred (new grads considered) Apply Apply online: tinyurl.com/sylvavalley
