Description Occupational Therapist About the Role PACE North is seeking a dedicated and compassionate Occupational Therapist (OT) to join our interdisciplinary care team. The OT will be responsible for designing and implementing individualized therapy programs that enhance independence, safety, and quality of life for participants. This role involves both clinical expertise and collaborative teamwork within our supportive care environment. Key Responsibilities Conduct initial and ongoing assessments of participants’ conditions and functional status. Develop and implement individualized treatment plans and goals. Record treatments and progress notes accurately in participants’ medical records. Assess and recommend adaptive or durable medical equipment (DME). Participate in interdisciplinary team meetings, case conferences, and care planning sessions. Communicate participant changes and updates to the team promptly. Contribute to Quality Improvement (QI) processes and initiatives.. Adhere to all PACE North policies, rules, and regulations. Perform additional duties as assigned. Requirements Qualifications. Licensure/Certification: Licensed Occupational Therapist in the state of Michigan Current CPR certification Valid Michigan driver’s license (travel required) Experience: Minimum 1 year experience working with a frail or elderly population (2+ years preferred) Skills: Demonstrated clinical competency with geriatric participants Basic computing proficiency Ability to work within PACE North’s established scope of practice and competencies Why Join PACE North?
