MAIN DUTIES OF THE JOB

You’ll be responsible for assessment, planning and implementation of care, and assist in the interpretation of information relevant to the patient’s condition/progress, identify problems and needs and set realistic rehabilitation goals. You need excellent organisational skills.
You will need to be able to provide specialist evidence based treatment plans for adults within the catchment area, either in their own homes or care homes.
You will work alongside the wider multidisciplinary teams within the community which includes partner agencies, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services
This post covers the Penwith are which spans from Hayle to Marazion and all areas to the West.
The service covers a 7 day rota which involves some weekend and bank holiday working: the successful candidates must be flexible with their working days. The service operates between the hours of 8am-6pm
