In this role, you will support patients in regaining their independence and improving their quality of life within the comfort of their homes. QUALIFICATIONS Required * Master's degree in Occupational Therapy * Valid state license or certification as an Occupational Therapist * Previous experience in case management is preferred * Strong communication and interpersonal skills * Ability to work effectively in a team environment * Excellent problem-solving and critical-thinking abilities  Preferred * Experience in home health or rehabilitation settings * Experience working with geriatric or post-acute care patients * Strong documentation and patient education skills WHAT YOU’LL DO * Conduct assessments to evaluate patients' physical, emotional, and cognitive abilities * Develop individualized treatment plans based on assessment results * Provide occupational therapy interventions to improve patients' functional abilities * Monitor and document patients' progress and adjust treatment plans as needed * Collaborate with other healthcare professionals to coordinate patient care * Educate patients and their families on therapeutic techniques and strategies COMPENSATION Hourly Rate $70 - $80 an hour APPLY TODAY!
