Your Opportunity Occupational Therapist registration required Based at Penrith - Full time (12 month contract) - Monday-Friday - $59.65 per hour As our Restorative Care Partner, you will manage the Restorative Care Pathway (RCP) program, a high-intensity, short-term, goal-oriented intervention to help older people improve health, delay functional decline, and stay independent for longer. With integrity and commitment, you will: Conduct clinical assessments and evaluations within your professional scope Develop, implement and review tailored care plans with internal/external health professionals Maximise services and resources to meet client goals within budget Support clients to transition into ongoing care after the program for a seamless experience Maintain accurate, timely documentation in CRM and related systems What You’ll Bring You are a compassionate and skilled allied health professional with a strong commitment to making a difference for older Australians. You will also need: Occupational Therapist qualification (University level) Current AHPRA registration Experience in aged care or community settings conducting comprehensive assessments Current driver’s licence and car Proficiency with technology and client management systems Ideally, you will also have: Experience collaborating with multi-disciplinary teams Strong communication and problem-solving skills A commitment to Anglicare’s Christian vision, mission and values Why Anglicare?
