Description

The Community Chronic Conditions Service (CCCS) aims to prevent morbidity associated with chronic and complex conditions, prevent hospital admissions, reduce length of stay and prevent premature admission to Residential Aged Care. The CCCS is part of Extended Care Services (ECS) in the Community and Preventive Health (C&PH) Directorate.
CP&H is a diverse group of service units delivering care and interventions outside the acute hospital setting in most cases. These services comprise all aspects of the care and life continuum including population health, prevention, early detection, management, reducing disease progression and complications, Aboriginal and Torres Strait Islander health care, rehabilitation and end of life care.

Responsibilities
  • Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery.
  • Provide leadership oversight and clinical practice supervision to less experienced Occupational Therapists practitioners and students as delegated.
  • Adhere to relevant expectations outlined in the SCHHS and Allied Health Governance Framework documents and Business Rules to ensure that best patient outcomes are achieved in a transparent, ethical and accountable manner.
    Remuneration value up to $12 731 p.m., comprising salary between $4769.60 - $5132.40 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (temporary position available until 30 June 2024).
