We are deeply rooted in our mission of being a FORCE for Good, and we’re looking for an Occupational Therapist (OT) to join our growing, tight-knit therapy team of five superstar clinicians. Our facility features a well-equipped therapy gym and private Occupational Therapy Suites, creating an ideal setting to make a meaningful impact on patients' lives. As an Occupational Therapist, you will assess patients’ functional abilities and develop personalized treatment plans to support their rehabilitation goals. Under physician direction, you’ll provide hands-on therapy, document progress, and educate patients and families on effective techniques to promote recovery and independence. Essential Functions: Evaluate, treat, and oversee therapy services for residents based on referrals and patient needs. Follow MDS procedures and contribute to effective care planning. Document progress and communicate with patients, families, and care teams. Supervise and delegate appropriate tasks to COTAs and Rehab Aides per regulations. Support skill development of therapy staff and foster collaboration within the interdisciplinary team. Participate in patient care meetings, utilization reviews, and family conferences. Ensure regular, on-time attendance for all scheduled shifts. Deliver occupational therapy services per physician orders. Continuously assess and adjust therapy plans to meet patient goals. Perform additional duties as assigned. What You Will Bring: Must have graduated from an accredited school of occupational therapy. Current OR license as an Occupational Therapist. Current CPR certification. Six months experience in a long term care environment preferred. Strong communication skills with patients, families, and facility staff. Ability to maintain confidentiality and uphold patient privacy standards.
