Responsibilities

As a Reablement Occupational Therapist, you’ll play a vital role in helping individuals regain independence and confidence in their daily lives following illness, injury or a decline in function. Working in people’s homes and community settings, you will deliver high-quality, client-centred reablement interventions to a varied caseload – from older adults to those recovering from hospital discharge, surgery, or managing long-term conditions.
You will carry out holistic functional and environmental assessments, set personalised reablement goals, and provide time-limited, targeted interventions that promote self-care and reduce dependence. This includes recommending and prescribing equipment, advising on adaptations and offering practical training to clients, carers and Community Reablement Assistants.
Collaboration is key – you’ll work as part of a multi-disciplinary team alongside Social Workers, Reablement Officers, health professionals and families to ensure clear pathways, SMART goals are met and risks are managed.
