What You'll Be Doing You'll deliver person‑centred functional and environmental assessments and interventions that empower clients to maintain independence and improve quality of life. Working in clients' homes, you'll provide specialised equipment solutions and pressure care strategies. You'll also be responsible for: Managing a wide variety of client presentations across multiple community aged care funding programs including Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care. Conducting in‑home assessments and developing tailored treatment plans to help clients meet their goals. This includes simple and complex equipment prescription, major and minor home modifications, and client education. Collaborating within a supportive statewide Occupational Therapy team of 70+ OTs with decades of collective experience, as well as working locally with a diverse multidisciplinary team of nurses, care workers and support staff.
