As one of the Occupational Therapy Aide in the service, you will lead the way to ensuring the best possible outcomes for young people aged 14-25 years in our service, supporting and empowering young people and their carers by developing independent living skills at the right time through an Occupational Therapy lens.
You will enable young people to achieve positive outcomes and independence by supporting the development of occupational performance skills, prescribing equipment, minor adaptations, and supporting within our Early Help model in the Transitions hub.
You will enjoy supporting a consistency of approach in the completion of support conversations, Care Act assessments and support plans leading to the commissioning of services to meet identified need where non-commissioned support, assistive technology, equipment, or wider community assets to ensure independence outcomes.
In Bristol, we are committed to hearing the young person’s voice, and we have a newly created Standard Operating Procedure (SOP) for our Transitions Hub model.
