We are seeking an Occupational Therapy Assistant (COTA) to join our therapy team at Grand Village in Grand Rapids, Minnesota. This role provides occupational therapy services to adult and geriatric patients in a skilled nursing and long-term care setting. This is an immediate opening with a strong hourly rate and leadership support. What You'll Do Provide skilled occupational therapy services under the direction of a licensed Occupational Therapist Implement individualized treatment plans focused on functional independence Treat adult and geriatric patients in skilled nursing and rehabilitation settings Monitor patient progress and communicate updates to the supervising OT Collaborate with interdisciplinary team members Complete timely and accurate documentation What We're Looking For Graduate of an accredited Occupational Therapy Assistant program Current Minnesota COTA license (or eligibility) Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred Strong communication and clinical reasoning skills Why Join Blue Stone Therapy?
