Job Description: All Care Home Health is currently seeking a Licensed Vocational Nurse (LVN) Care Coordinator to join our team supporting the Kaiser region. We are looking for a motivated and organized LVN who can help coordinate patient care and ensure smooth communication between clinicians, patients, and the office. Newly graduated LVNs are welcome to apply. Candidates with care coordination or case management experience are highly preferred. Responsibilities: * Coordinate patient care and services for home health patients. * Communicate with field clinicians regarding patient needs and care updates. * Monitor and verify patient authorizations and visit schedules. * Ensure timely documentation and compliance with agency standards. * Collaborate with physicians, nurses, and interdisciplinary team members. * Assist with care planning and coordination of services. Qualifications: * Active California LVN License (Required). * Strong communication and organizational skills. * Ability to multitask and coordinate patient services efficiently. * Experience in care coordination, case management, or home health is a plus. * Newly licensed LVNs are encouraged to apply.
