Description

We are seeking an older adults and dementia nurse to join our mental health liaison team based at the Princess Royal Hospital in Haywards Heath, this is a 9-5 Monday-Friday role.
You will be working at the interface between physical and psychological health and bring your knowledge and expertise to your general hospital colleagues. You will work within our MDT in order to provide specialist assessment, treatment and discharge planning to those patients with functional and organic mental health conditions.
We support patients referred to our service with a wide range of physical and psychological co-morbidities including those people presenting to mental health services for the very first time.
You will need to be able to work autonomously and have excellent communication skills and be able to foster excellent relationships with patients, carers, general hospital colleagues and other parts of the health and social care system.
We are Sussex Partnership NHS Foundation Trust, providing mental health, learning disability and neurodevelopmental services to people living in south east England. Our services are for children, young people, adults of working age and older people.
This is a challenging but rewarding role, offering an exciting opportunity. You will need the ability to work autonomously whilst still contributing to the team as a whole. If you enjoy the challenge and stimulus of working under pressure and are able to remain calm in quickly changing circumstances, you will be well suited to this role.
You will be independently undertaking comprehensive assessments for older adults and people living with dementia, working autonomously to generate risk assessments, care plans, and onward planning. This will involve working with patients, their loved ones, other areas of mental health services, our acute trust colleagues and more to ensure positive outcomes for all those referred to our service
