Job Purpose: As a Registered Nurse (RN) Case Manager specializing in hospice at Lifted Hospice, you will play a pivotal role in providing patient-centered care with a strong emphasis on compassion and dignity. Your responsibilities will include administering skilled nursing care for clients of all ages in their place of residence, coordinating care with the interdisciplinary team, patient/family, and referring agency, and assuming responsibility for the coordination of care. Duties & Responsibilities: Comprehensive Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs, tailoring care plans to meet individual requirements. Expert Hospice Care: Provide expert hospice care and symptom management to patients, ensuring their comfort and enhancing their quality of life during this sensitive time. Collaborative Approach: Collaborate closely with the interdisciplinary healthcare team, patients, and families to ensure holistic and comprehensive care. Meticulous Documentation: Maintain accurate and comprehensive medical records, adhering to regulatory guidelines and standards. Emotional Support and Education: Offer compassionate emotional support and education to patients and their families, helping them navigate the challenges of end-of-life care. Skills and Qualifications: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing. Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license. Experience as a registered nurse in a clinical care setting. Hospice experience preferred. Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English. Reliable transportation and valid auto liability insurance. Working Conditions: The worker is subject to both environmental conditions: Activities occur inside and outside. The worker is exposed to infectious diseases and bodily fluids. Physical & Mental Requirements: Requires prolonged sitting and use of office equipment; may involve lifting, bending, and standing. Ability to lift up to 50 pounds and assist with patient transfers when necessary. Must be able to manage stressful situations calmly and professionally. Requires quick, sound decision-making in clinical and administrative contexts. Must work efficiently under deadlines and pressure to meet patient and organizational needs. Requires eye-hand coordination and manual dexterity.
