WORK EXPERIENCE:

• 2 years working with geriatric populations preferred, but not required.

SKILLS:

• Ability to read/write in english
• Bilingual (Tagalog or Spanish)preferred, but not required

EDUCATION:

• MUST have bachelor’s degree in the social sciences or related fields

• Valid CPR/First Aid
• Criminal Background Clearance
• Copy of Negative TB result
• Copy of HEP B Vaccination
• CA Drivers License

• Assisting center’s social worker with the following:
  Quarterly Notes
  Individualized Plan of Care
  Assisting participant needs w/ housing, finances, healthcare, etc…
  Documenting daily flow sheets pertaining to participant
  Setting up multidisciplinary team meetings or participant in-services
  Assisting Program Director as needed
