Description

Serves as an expert oncology population specific clinician, patient/client advocate and assists cancer/oncology patients/families to navigate the complex health care system in an organized, effective and efficient manner. This position will assist with the standardization and efficiency of care for patients in the cancer service line throughout the continuum of patient care. The Oncology Social Work Navigator models patient-centered care and assumes leadership responsibilities with high level contribution in direct patient interaction, the department, the service line, and across the institution.
1. Navigate Cancer Patients Through the health Care System
a. Initiates communication with patients upon learning they have an abnormal or suspicious finding. The navigator will begin to guide the patient through the health care system by assisting them with scheduling follow-up appointments and ensure that they arrive on time and are prepared.
b. Address any barriers the patient may experience that would prevent them from receiving appropriate follow-up. c. Links patient to community and social support services as deemed appropriate.
d. Facilitate interaction and communication with health care staff and providers as needed.
e. Addresses the patient’s psychosocial needs as well as provide initial counseling.
f. Provide health education to individuals and groups.
2. Develop Resources to be Used in Patient Navigation
a. Identify key personnel in all partnership facilities and departments that are involved in the care of patients and develop and maintain relationships with them.
b. Utilize existing referral and follow-up procedures as appropriate and refine procedures as necessary.
c. Identify gaps in resources and work within the community to fill those gaps.
d. Develop and maintain library of resources and disseminate information to patients; develop brochure describing the program
e. Develop and maintain a learning library for patients that includes culturally specific and language specific materials
f. Assist the patient with transportation as needed.
3. Reporting and Evaluation of Patient Navigation Program
a. Provides reports and statistics as necessary to assess progress of program
b. Maintain accurate, confidential records documenting services provided, contacts and unmet needs.
c. Maintain patient confidentiality in accordance with HIPAA regulations
d. Provides follow up to all patients with abnormal findings from screenings, including face to face meetings, phone calls, certified letters, etc.
Experience 2 years overall social work experience required
Experience in oncology social work or a related field highly preferred
Education Master’s Degree in Social Work or Counseling Required
Special Skills Proficient in Psychosocial assessment.
Focuses on patient safety
Intentionally communicates and engages patients and caregivers to ensure their needs are being met.
Advocates for patients and their families in overcoming barriers to healthcare by facilitating needs, education, resource provision and/or representation within the multi-disciplinary care environment
Licensure Active Louisiana LCSW Social Worker license required
