The Out of Home Care (OOHC) Health Pathways Program is a key initiative arising out of the Keep Them Safe: a Shared Approach to Child Wellbeing 2009-2014, which is the NSW Government Response to the Report of the Special Commission of Inquiry into Child Protection Services in NSW.
The OOHC Liaison Nurse facilitates and implements health assessments and health management plans for children and young people entering Out of Home Care (OOHC) within Western Sydney Local Health District (WSLHD).
The position will work within a multidisciplinary and multi-agency approach with key stakeholders both internal to Integrated & Community Heath Directorate and within WSLHD, and externally with key government and non-government partners including Department of Family & Community Services – Community Services, Department of Education & Communities, Western Sydney Primary Health Network (WentWest), General Practitioners, private health care providers, non-Government organisations, as well as children and young people in out of home care and their carer’s. The position has clinical, liaison, implementation and monitoring functions.
