QUALIFICATIONS

• Valid state licensure as a Physical Therapist (PT) or Physical Therapist Assistant (PTA).
• Knowledge of anatomy and physiology relevant to physical therapy practices.
• Experience in various settings such as geriatrics, pediatrics, orthopedics, and skilled nursing facilities is preferred.
• Familiarity with discharge planning processes and care plan development for diverse patient populations.
• Strong communication skills to effectively interact with patients, families, and healthcare professionals.
• Ability to work collaboratively within a team-oriented environment while maintaining a focus on individual patient needs.

• Conduct comprehensive assessments of patients to determine their physical therapy needs.
• Develop and implement personalized treatment plans based on individual patient goals.
• Utilize various therapeutic techniques including trigger point therapy to alleviate pain and improve mobility.
• Educate patients and their families on treatment plans, exercises, and rehabilitation techniques.
• Collaborate with multidisciplinary teams to ensure cohesive patient care across inpatient, outpatient, and acute care settings.
• Maintain accurate documentation of patient progress and treatment outcomes in accordance with medical terminology standards.
• Participate in discharge planning to facilitate smooth transitions for patients leaving skilled nursing facilities or acute care environments.
• Stay current with advancements in physiotherapy practices and integrate new techniques into patient care.
