Job Details Job Location: Valley Glen, CA Salary Range: $23.00 - $28.00 Hourly Description Duties and Responsibilities: The Outreach Specialist will perform strategic outreach to community residents who lack access to healthcare and health insurance, provide follow-up assistance and build/strengthen enrollment collaborations with local partner organizations to increase health insurance coverage and access to quality primary health, dental, and behavioral care for identified community residents. The Outreach Specialist will also support Via Care’s partnership with the Los Angeles Department of Water and Power (LADWP). Essential Functions: The functions performed by employees in this job family will vary by level or the area(s) to which assigned but may include (and not be limited to) the following. Employee must perform all duties and responsibilities in accordance with Via Care’s care standards. • Provide structured patient education on health coverage and resource conservation and energy efficiency. • Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness. • Develop, discover, and attend community events to promote coverage options and the mission and services of the organization. • Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages. • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities. • Accurately provide required reporting to track goal achievement and client satisfaction. • Assist in the development and implementation of organizational outreach and enrollment initiatives. • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to all policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access; and • Perform other duties as assigned. Knowledge, Skills, and Abilities: • Knowledge of the health and human services infrastructure, health insurance programs and public coverage options. • Ability to effectively develop and nurture relationships with a diverse group of stakeholders. • Ability to work independently and coordinate multiple tasks. • Ability to enthusiastically communicate with potential enrollees. • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications, and the Microsoft operating system. • Ability to manage time and meet deadlines. • Excellent verbal and written communication skills. • Ability to maintain accurate records and necessary paperwork. • Ability to learn and apply training instruction. • Ability to maintain strict standards of confidentiality regarding patient information. • Ability to maintain a calm professional demeanor and make independent judgments. Qualifications Qualifications: • Two years’ experience working in a community social service or healthcare environment. • Covered CA Enrollment Assister Certification preferred. • Current HIV/STI Counseling Certificate preferred, as well as current training on family planning practices and methods. • Bilingual in Spanish and English required. • Must have excellent reading and comprehension skills. • Must be able to work efficiently in a very fast-paced environment. • Must adhere to the agency’s non-discrimination policies. • Experience working with ethnically and culturally diverse communities, and lowincome populations. • Ability to effectively interact with co-workers and patients with diverse ethnic backgrounds, religious views, political affiliations, cultural backgrounds, lifestyles, and sexual orientations and treat individuals with respect and dignity
